Tooth extraction while on warfarin/coumadin

Hi guys,
Just want to know if anyone of you had experienced tooth extraction while on warfarin/coumadin? Is the risk of bleeding too high?
Because I have a severely painful tooth which I wanted to be extracted asap. I am on my 3rd month of warfarin. My hematologist wants me to finish to 6 months.
Thanks!
Michael

Hello, while on warfarin I too was due to have wisdom teeth removed, I'm sure of the correct procedure, but at the time I decided to dealy the dentist visit on advice from a family member. I'd imagine that the blood being quite thin can effect blood loss, but not positive. I would call the dentist and ask if your unsure.

Had some dental work on the gum line and my Dentist had no problem with bleeding, INR was 2.4. Some other Dentist's might want you to omit the day before dose; I did that for laser eye surgery and no clotting problems. Ask your Dentist how he wants to proceed and how he will control bleeding.

I was told if some procedure like that was needed you might have to come off the medicine for a couple of days. It will be a real good thing to consult both your doctor most importantly, and your dentist.

I agree with Justin - your dentist needs to know you are on coumadin - I would assume he wouldn't want to do invasive surgery if you're on coumadin unless it was an emergency.
You might ask your doctor if you can stop taking coumadin for a few days to drop your INR - that way you won't bleed so much after the proceedure.
I've been told they won't even give Botox injections if you are on Warfarin, for fear of bleeding and bruising. Same with piercing ears. As a botox injection is a lot smaller than a tooth extraction, there's got to be some issues involved.

I have had many teeth removed while on coumadin. My oral surgeon says I just need to get down to around 2 on my INR. I have had no problems at all. I just had a tooth removed a month ago.

I am a dental assistant, we always tell out coumadin/warfarin pt's to stop coumadin 5 days prior to the extraction, unless told by gp not to, there is a much increased risk of bleeding if you stay on it & have the tooth out.

I had 5 teeth removed on July 12 and had no bleeding problems, omitted my Warfarin 5mg for only one night. All healed and waiting for my new teeth, hate the toothless smile. :-)

Michael:

The current standard for a simple tooth extraction while on warfarin is now no disruption of anticoagulation therapy...that disruption of anticoagulation therapy produces more risks than bleeding risks from simple tooth extractions. If you want to read more there is a very good article on this at www.clotcare.com

I am going to have two teeth removed Friday. The oral surgeon just wants me to get to around 2 on my INR. He also believes that it is not a good idea to stop the therapy. I always test at 2.4 to 2.6 so I will only be stopping my warfarin for 2 days.

Same advice as Will... 2 is the goal, dentist asked for a reschedule if over 2.4 INR the
day before. They have the equivalent of a styptic pencil embedded in gauze for dental
procedures, you can imagine what it tastes like but it works. The real pain is that
NSAIDS can do more than vicodin to ease pain associated with dental stuff, but all
we've got is tylenol or vicodin as a leg up.

I am a Dentist. As long as your INR is kept between 2.0 to 2,2 you will be fine. Always check with your GP and Dentist first.

My heart Dr. wants me to be off of my coumadin for 5 days and start back the next day after the tooth is pulled. I think that is scarry to be off for 5 days as I have Atrial Fibrillation and really need to have my blood thin, in case of an attack I could have a stroke. I think being off 5 days is too long after reading yours, maybe 2 days.

I just had a tooth removed up top and have been on Warfarin for over a year now (lifetime therapy due to 2 lifetime clots in my legs). My Oral Surgeon said to maintain my current INR but paranoia made me reduce my meds slightly. I had the procedure done today, my INR was 1.7 which was a bit low but started taking the regular dosages following the procedure. Bleeding was minimal of course because of the lower INR.

What about regular cleaning while on coumadin? Next week will be first since pe
and they always do a lot of digging around gums.

I am a dental assistant and you do not need to stop your anticoagulant for cleanings
nor extractions, or Any other kind of dental procedure for that matter x

Hi, I am on lifetime warfarin for 5 years now due to synthetic heart valve. I am going to have my wisdom tooth removed after it got an inflammation. I am sure that bleeding can be managed by the surgeon adequately during the procedure, no matter what my INR is. What troubles me is that I read on a dentistry forum that wisdom teeth extraction leaves a large cavity which must be filled with a blood clot and stay that way in order to heal well.

My INR must be kept around 2.5 and the dentist told me not to omit any doses (cause of the serious heart risk).

So, is that clot going to form? is it going to heal ok?
